Enrollment Procedure:

To enroll and reserve a space in an Underwater Arkansas program please click on the link below and print, complete, sign and mail the documents along with payment to Kirsten Bartlow at the address below. PAYMENT and DOCUMENTS must be received at our office to be considered confirmed. Please note that space is limited and available on a first come first served basis by received date. If a reservation is received for a program that is full, the participant will be contacted immediately and transferred to another program or refunded.
